IMPORTANT NOTE:

When submitting the form either Electronically or via PDF, the confirmation you receive from the WSO must be emailed to pghaissecretary@gmail.com in order for the meeting changes to appear in the Meeting List, The Key and AIS Website. The WSO does not notify the AIS of any group changes.
